By participating in a class, event, or program with Evelyn's Corner, you are agreeing to the following:

In consideration of my involvement with Evelyn’s Corner, LLC. , I, and/or my minor child (collectively, whether one or more, “We”) hereby give permission to participate in all art related programs provided by Evelyn’s Corner, and we hereby release and save harmless all employees, instructors, and volunteers associated with Evelyn’s Corner and Evelyn’s Corner from any and all liability and claims which may arise from my and my child's participation in art classes and/or other events provided by Evelyn’s Corner.

I give permission for Evelyn’s Corner or officers, members, managers, agents and other affiliated persons or entities of Evelyn’s Corner, to take whatever steps are reasonably necessary to obtain emergency medical care, if required. Any expenses incurred in carrying out these steps will be borne by the child’s family.

I understand that our photograph or likeness may be published to social media and/or the Evelyn’s Corner website, or other promotional material unless I have indicated otherwise.   

Full payment is required by the start of each class or event. Classes that are cancelled due to inclement weather or instructor illness may be rescheduled. If you need to cancel a class for personal reasons, you must do so by phone or email no later than 24 hours before the class. No refunds will be given for late cancellation of a class. Camps are non-refundable, but may be eligible for transfer or credit.  If you need to cancel a camp, you must do so 7 days prior to the camp to receive a credit or transfer.  Late cancellations are not transferable or eligible for credit.

I have carefully read and freely signed this Release of Liability and User Agreement. To the best of my knowledge, all information provided above is accurate, and unless otherwise noted in detail on this form, my child has no physical, emotional or social conditions which make it dangerous for him/her to participate in art program activities with Evelyn’s Corner. If any of the information I have stated on this form about my child changes, it is my responsibility to provide Evelyn’s Corner with those changes. I understand and agree that no oral or written representations can or will alter the terms of this document. I agree that this agreement shall be governed by the laws of the state of Pennsylvania.
